Community Care

A shift to enhanced care in the community is widely supported as the way forward when it comes to improving access to care for all, better work-life balance for health care workers, and better health overall.

Here in Newfoundland and Labrador, Family Care Teams are the vision for improved community care. They are multi-disciplinary teams that work collaboratively to best meet the needs of patients and families. These teams include family physicians, nurse practitioners, nurses, pharmacists, social workers, as well as other allied health professionals.

Quality of Care NL supports this transition by investigating barriers and enablers to team-based primary health care to help inform the implementation of Family Care Teams.
